Issue: Regarding customizing Form Manager plugin.

Brief Description of Issue: I have downloaded 'Form Manager' Plugin and its awesome.

But I need some customization to suit my requirement. I need the submission to be posted in a thread. My issues are:

1. I need a customized thread name of the submission which is not default title "Form Submission: <form name>". I need it to be from a text field of user input.

2. I need the output layout to be changed, like I don't need that <hr> lines, I need a fixed sentence with some variable words taken from user input etc. If a .php is generated during creation of the form I need to know the location. If I can have that custom form php I can edit as per requirement after creating the form.

Nearly a necro-post here but, are you unable to resolve some of issues by tweaking the underlying code yourself? Perhaps you accomplished that already?

This specific Mod is what got my attention with this site, so I'm curious if you resolved your issues on your own?
