reCAPTCHA is great against spam bots.
I usually chip in to reCAPTCHA with a Mathematical Question as well as an Anti-Bot question, and I never see the bloody things again. Doesn't account for human spammers though, which register accounts on my website every couple of days with 0 posts and just advertisements in their profile for their own crap. But those are easily covered by some of the methods presented above.

Thats what almost every forum uses and its not so safe anymore, because the spammers usually have about a dosen emails registered, because you can have unlimited emails registered with MSN, Gmail, Yahoo etc. However, its good enough to prevent false email registrations
