You need to edit the templates to your theme
look in the Affiliates Manager docs section and open the read me. In there you will see that you need to do some template edits.

You need to go to the acp>templates> and then you should see "Default Templates" and then "Used by: yourtheme"

do the template edits and you are set to go.
