For some reason, it's not letting anyone but administrators view or fill out the form... this is getting a bit annoying.

Yes, the permissions are set to allow everyone accept banned members and guest fill it out. There are is no post requirements, the maximum uses are unlimited and it is active.

Can anyone help me figure out why its not letting anyone fill it out?

Has anyone has issues with this in MyBB 1.6? I am having issues with it now, on a freshly installed version of 1.6.
