I answered one of my own questions. Form Manager does not work on 1.8 as of 04/02/2015. The installation completes the menu item shows up but does not function.

I would still be interested in your beta program if your are looking for more users.

I installed it today and activated it. When I tried to configure the link errored out. I cannot provide the exact error do to Godaddy hosting.
