Form Manager Tutorial
Form Manager is a mod created by Nickman for MyBBSource. It is a very simple and easy way to let you create forms for your users to answer. Whether they would be a staff application form or just a questionnaire it’s up to you. Once you have installed the form manager it is located in the Configuration tab under the link ‘Form Manager’.

To add a new form click the button in the top right hand corner as shown above. This will bring you to a page where you can configure the name of the form, the permissions and different requirements to be able to use the form. (Post count and maximum uses) You can also specify where the results of the form go, such as into a PM or a forum. You can see a full screenshot of the ‘add new form’ page below.

Once a form has been created you can see it in the created forms page shown below.

From here you can delete and edit the form but the most important part of a form is the items inside it. Once you have clicked the Manage Items link you will come to page with a link in the top right corner named ‘Add new Item’. If you click this then it will take you to a page where you can choose what type of form object you want/need. (Options below)

Once you have chosen your option it will take you to the settings page of the item. It will ask you to specify the name, description, css class, default value, the max length and the size. (All apart from name are optional) You can also choose whether it is a required option or not. Different types of objects may have different settings (such as html block and captcha) but they are generally the same options. The settings page for a textbox is shown below.

Once you have created the object it will appear in a list where you can change the display order and go back to edit the object if you wish. (Shown below)
