Ok, this is quite possibly one of my COOLEST mods yet. What it does is allow you to Create a form.

When creating a form, you have these options:

• Permissions (Which groups can submit this form?)
• Required Post Count ( how many posts must they have before they can submit form)
• Active/Not Active
• How many times can a user submit this form
• UID to PM results to
• Forum to post results in
• Emails to send results to
  

Once you've done that, you can add "Items" to the form, which are things the user has to fill out.

For these you can use:

• Input box
• Textarea
• Drop Down Select
• Multiple Select
• Radio Buttons
• Checkboxes
• HTML Block (your own content)
• Separators (Separate different sections of the form)
  

You can also add a captcha and set the groups that will have to fill it out before posting.

That is one SEXY mod, I dont think i need it but im going to think of a way to use it because it looks cool.

Once I have made a form the forms.php page still says Form not found even when i have enabled the form.

You have to go to the id..so forms.php?id=1

and your second form would be forms.php?id=2 and so on.

For the

Emails
Who would you like to be emailed with the results? Separate with a comma.

What do i need to put, the persons email, uid, or username ?

email addresses...

Like:
[email protected],[email protected] ect

Each one will receive an email with the results

Very nice. Does this include CAPTCHA in the form (if selected, ofc)?
